This statutory declaration is made pursuant to the Statutory Declarations Act 1835 (c.62). The declaration must be made before a practising solicitor, commissioner for oaths, notary public, or justice of the peace. Making a false declaration is a criminal offence under section 6 of the Perjury Act 1911, punishable by up to two years' imprisonment. The Enrolment of Deeds (Change of Name) Regulations 1994 (SI 1994/604) provide for optional enrolment at the Royal Courts of Justice. For changes of name following gender reassignment, the Gender Recognition Act 2004 provides a separate legal process for obtaining a Gender Recognition Certificate. This template is valid for use in England and Wales. Scottish law recognises statutory declarations under the same Act.
